Refer a Patient to Our Bakersfield and Valencia Oral Surgeons

We treat every referral as a shared case. Your patient comes back to you with the surgical phase complete and a written record of what was done.

Four commitments,
and we keep them.

We treat every referral as a shared case. Your patient comes back to you with the surgical phase complete and a written record of what was done.

A referral is a transfer of trust, not just a transfer of a chart. The surgical phase happens here, the documentation follows the patient, and the restorative work stays where it belongs, with you.

01

Reports that arrive

A consultation summary and an operative note go out for every patient, so the case is documented on your side as well as ours.

02

Urgent cases seen urgently

Call the office directly for acute infection, trauma or an unexpected surgical finding and we will make room the same week wherever the schedule allows.

03

Your restorative plan leads

For implant cases we plan backwards from the restoration you intend to place. Tell us where the crown needs to sit and we will put the fixture where it belongs.

04

The patient returns to you

We do not restore. Surgical phase complete, the patient goes back to your chair.

Five things, and we can start.

Send what you have. A missing film will not hold up a consultation, but the more that arrives with the referral, the fewer calls your front desk fields afterwards.

  1. 01Patient name, date of birth and contact number
  2. 02Reason for referral and the tooth or site in question
  3. 03Any recent radiographs, including a panoramic film or CBCT if you have one
  4. 04Relevant medical history, current medications and known allergies
  5. 05Your preferred timeline, and whether the case is urgent

Three ways to reach us.

Referral form

Enough to open the case and contact the patient. Our office handles the scheduling from there.
